Discharge Reasons

There are many reasons to truly understand the satisfaction levels of patients upon discharge.

How was their overall satisfaction with their service?
How well are they adjusting to their new surroundings?
Would they recommend your services to others?
Do they need any additional services?


A high quality feedback system must find out this very valuable information.

Another valuable tool in our discharge feedback system is what we call a 'Focus Interview'. More and more we are being asked to conduct 'Focus Interviews'. At times, facilities/agencies will have patients that leave service before their specific needs are met. This can leave some unanswered questions as to why the person left service so abruptly. Our 'Focus Interview' has proven to be a non threatening yet direct tool for our clients to obtain this very important discharge feedback.
